On December 11, 2003, Gaston G. Guerra, M.D., Surgical Practice P.C., filed its Articles of Dissolution with the Missouri Secretary of State.

You are hereby notified if you believe you have a claim against Gaston G. Guerra, M.D., Surgical Practice P.C., you must submit a summary in writing of the circumstances surrounding your claim to the corporation at 8207 W. 99th Street, Overland Park, KS 66212 ATTN: Gaston G. Guerra, M.D. The summary of your claim must include the following information:

Be advised that a Notice of Winding Up was filed with the State of Missouri on December 24, 2003 for R & H, L.L.C. In order to file a claim with the company, persons with claims against R & H, L.L.C. must furnish: (1) amount of claim; (2) basis for claim; (3) documentation of the claim. On December 30, 2003, the Lake Winnebago Land Company, Inc., a Missouri corporation, filed its Articles of Dissolution with the Missouri Secretary of State. The dissolution was effective December 31, 2003.

Twain Haven, Inc.

Take NOTICE that Twain Haven, Inc., a Missouri not-for-profit corporation, formerly doing business as Twain Haven Care Center, Perry, Missouri, filed Articles of Dissolution with the Missouri Secretary of State which were effective on December 9, 2003.

Any person with a claim against such corporation must file a claim in writing which must include the full name, address and telephone number of the claimant, the amount of the claim, the date the claim accrued, whether the claim is secured, and if so, the collateral, and describing the claim and the account on which it is made with specificity.

This notice is to inform whom it may concern that National Avenue Building Company has on the 19th day of December, 2003, filed with the Missouri Secretary of State the corporation's Articles of Dissolution. Dissolution was effective on December 31, 2003.
