Notice is given that Cardiopulmonary Rehabilitation Consortium of Greater Kansas City, a Missouri Nonprofit corporation, f/k/a Cardiac Rehabilitation Consortium of Greater Kansas City, with its registered office at 1000 Carondelet Drive, Kansas City, Missouri 64114, has been dissolved as of December 31, 1999 in accordance with the Missouri general corporate code. Cardiopulmonary Rehabilitation Consortium of Greater Kansas City requests that persons with claims against the corporation present the claims in accordance with the Missouri general corporate code. The claim must include the name of the claimant, the claimant's mailing address, and information describing the claim with specificity.
