On November 14, 2000, LCS MANAGEMENT COMPANY, a Missouri corporation, filed its Articles of Dissolution with the Missouri Secretary of State. Dissolution was effective on October 31, 2000.

Said corporation requests that all persons and organizations who have claims against it present them immediately by letter to the corporation at:

Please Take Notice that HAL PROW MEDICAL ELECTRONICS, INC. filed its Articles of Dissolution with the Missouri Secretary of State, effective November 22, 2000. Any and all claims against HAL PROW MEDICAL ELECTRONICS, INC. should be sent to Kent D. Kehr, 1010 Market Street, Suite 1300, St. Louis, MO 63101. Each such claim should include the following: the name, address and telephone number of the claimant; the amount of the claim; the date on which the claim arose; and a brief description of the nature or the basis for the claim. On December 15, 2000, Ball Clinic, Inc., a Missouri Corporation, filed its Articles of Dissolution with the Missouri Secretary of State. The dissolution became effective December 15, 2000.

All potential claimants or creditors of the Corporation are hereby notified that if they believe they have a claim against the Corporation, they must submit a summary in writing of the circumstances surrounding their claim to the Corporation at 4301 Madison Avenue, #113, Kansas City, MO 64111. Said corporation requests that all persons and organizations who have claims against it present them immediately by letter to the corporation c/o Paul T. Lyon at 3300 East Geospace Drive, Independence, Missouri 64056.

All claims must include: the name and address of the claimant; the amount claimed; the basis for the claim; and the date(s) on which the event(s) on which the claim is based occurred.
