Meramec Infusion, Inc., a Missouri corporation, filed Articles of Dissolution on July 6, 1998. This corporation did business under the name Health Management Solutions. Any claims against the corporation may be sent to W. H. Thomas, Jr., THOMAS, BIRDSONG & BECKER, P.C., P.O. Box 248, Rolla, Missouri 65042. Each claim must include the name, address and telephone number of the claimant, the dates of occurrence of events upon which the claim is based and a brief description of the basis for the claim or the nature of the debt, the amount of the

claim and whether the claim is secured, and, if so, the nature of the security. Any claim against Meramec Infusion, Inc. will be barred unless a proceeding to enforce the claim is commenced within two (2) years after the publication of this notice.
