Notice of Dissolution
of Limited Liability Company
To All Creditors of and
Claimants Against Med X Practice Solutions, L.L.C.

On June 28, 2004, Med X Practice Solutions, L.L.C. filed its notice of winding up with the Missouri Secretary of State. Dissolution was effective as of that date.
Said limited liability company requests that all persons and organizations with claims against it present them immediately by letter to the limited liability company c/o Van Allen, 2103 Burlington, Suite 900, Columbia, Missouri 65202.

All claims must include: the name and address of the claimant; the amount claimed; the basis for the claim; and the date(s) on which the event(s) on which the claim is based occurred.

NOTICE: Because of the dissolution of Med X Practice Solutions, L.L.C., any claims against it will be barred unless a proceeding to enforce the claim is commenced within three (3) years after publication date of the notice authorized by statute.


Notice of Corporate Dissolution To All Creditors of and Claimants Against
TLA, Inc.

On February 9, 1998, TLA, Inc. became administratively dissolved with the Missouri Secretary of State. Any claims against the corporation must be submitted in writing to John H. Schmidt, Schmidt, Kirby & Sullivan, P.C., 2838 S. Ingram Mill Road, Springfield, MO 65804. Each claim must include the name, address and telephone number of the claimant; the amount of the claim; the dates on which the claim arose; and a brief description of the basis for the claim. On June 25, 2004, Zubeck Davis, L.L.C. (the "company") filed Notice of Winding Up with the Missouri Secretary of State. Claims against the "company" must be mailed to: Donald L. Davis, 3419 Locust, Kansas City, Missouri 64109-2273.

Claims must include the name and address of the claimant, amount of the claim, basis for the claim, and documentation of the claim.
A claim against the "company" will be barred unless a proceeding to enforce the claim is commenced within three years after the publication of this notice.


Notice of Corporate Dissolution To All Creditors of and Claimants Against
S.M.B. Enterprises Inc.

Notice is given that Articles of Dissolution for S.M.B. Enterprises, Inc., a Missouri corporation with its office at 3409 Country Club Dr., Jefferson City, MO 65109, have been filed with the Secretary of State in accordance with the laws of the State of Missouri.

The corporation requests that all claimants against the corporation provide written proof of their claims to the corporation at the following address:

On July 26, 2004, J.A. Podhorn, Sr., Properties, L.L.C., a Missouri limited liability company (hereinafter the "Company"), filed its Notice of Winding Up with the Secretary of State, effective as of the date of filing by the Secretary of State.

The Company requests that all persons and organizations with claims against it present to them immediately, by letter, to the attention of: PHYLLIS J. ZERR, 1775 St. Peters-Cottleville Rd., St. Peters, MO 63376. Each claim must include the following information: the name, address, and telephone number of the claimant; the amount claimed; the date on which the claim arose; the basis for the claim; and documentation in support of the claim.

Notice of Corporate Dissolution To All Creditors of and Claimants Against
Long Auto Parts Inc.

On July 30, 2004, Long Auto Parts, Inc., a Missouri corporation, with offices at 204 Lee Street, Fredericktown, Missouri, filed its Articles of Dissolution by Voluntary Action with the Corporate Division of the Office of the Secretary of State.

All persons or organizations having claims against Long Auto Parts, Inc., are requested to present them immediately in writing to: Robin E. Fulton, Attorney at Law, 135 East Main Street, Fredericktown, Missouri 63645-0151.

Each claim must contain the following information:
(1) Name, current address and telephone number of the claimant;
(2) A clear and concise statement of the facts supporting the claim;
(3) The date the claim was incurred; and
(4) The amount of money or alternate relief demanded.

All claims against Long Auto Parts, Inc., will be barred unless a proceeding to enforce the claim is commenced within two (2) years after the publication of this notice.


Notice of Corporate Dissolution To All Creditors of and Claimants Against Provident Capital Resources Corporation

This notice is to inform who it may concern, that Provident Capital Resources Corporation has on the 31st day December, 2003, filed with the Missouri Secretary of State's office the corporation's Articles of Dissolution. Dissolution was effective on December 31, 2003.

Any claims against the corporation should be forwarded to Thomas R. Galganski, Attorney at Law, at the following address: 20 Allen Avenue, Suite 450, Webster Groves, Missouri 63119.

The claim must include the following information:
(1) The name, address and telephone number of the claimant;
(2) The amount of the claim
(3) The date the claim accrued or will accrue; and

You are further notified that all claims against the corporation shall be barred unless a proceeding to enforce the claim is commenced within two (2) years after the publication of the two (2) notices as required by Missouri statute, whichever is published last.
