On December 18 2002, GATEWAY PANEL & TRUSS, INC., a Missouri Corporation, filed Articles of Dissolution with the Secretary of State of Missouri. Dissolution was effective December 31, 2002. You are hereby notified if you believe you have a claim against GATEWAY PANEL & TRUSS, INC., you must submit a summary in writing of the circumstances surrounding your claim to: The Lowenbaum Partnership, L.L.C., c/o J. On April 10, 2003, Stainless Technology, LLC., a Missouri Limited Liability Company, filed its Articles of Dissolution with the Missouri Secretary of State. The dissolution of the Limited Liability Company was effective on that date. You are hereby notified that on March 31, 2003, Capital City Hotel Company, L.L.C., a Missouri limited liability company, agreed to dissolve and wind up the L.L.C. NHC, Inc.

The Board of Directors and shareholders of NHC, Inc., a Missouri corporation (the "Corporation"), have authorized the dissolution of the Corporation in accordance with Section 351.464 of the Missouri Revised Statutes. The Corporation is the successor-in-interest by merger to MidAmerica Equities, L.P. d/b/a Normandy Community Hospital, a Texas limited partnership. This notice is provided pursuant to Section 351.482 of the Missouri Revised Statutes, which states that any corporation that has been authorized to dissolve may provide written notice to all unknown creditors and claimants. If you have a claim against the Corporation, you should notify the Corporation in writing at 2200 Southwest Freeway, 5th Floor, Houston, Texas 77098. Such written notice should describe your claim in reasonable detail, state the amount of your claim and provide back-up documentation for your claim.
